Start by cleaning the fresh clams. Soak them in cold water with a little salt for about 20 minutes to remove any sand. Rinse them under cold running water. If using canned clams, rinse and drain them well.
Bring a large pot of salted water to a boil and add the spaghetti. Cook according to package instructions until al dente. Reserve 1/2 cup of the pasta water, then drain the spaghetti.
While the pasta cooks, finely chop the garlic and parsley. Heat the olive oil in a large pan over medium heat. Add the garlic and sauté until fragrant, about 1 minute.
Add the clams and white wine to the pan. If using canned clams, add them directly. Cover the pan and cook for about 5-7 minutes, or until all the clams have opened. Discard any clams that do not open.
Once the clams are cooked, add the red pepper flakes (if using), salt, and pepper to taste. Toss in the cooked spaghetti and a little reserved pasta water to help create a sauce. Cook for another 2 minutes until everything is well combined.
Remove from heat and stir in the chopped parsley. Serve immediately, garnished with additional parsley if desired.
