In a medium saucepan, combine 1 cup of sugar with 1 cup of water. Heat over medium heat, stirring until the sugar completely dissolves, creating a simple syrup. This should take about 3-5 minutes.
Remove the saucepan from the heat and let the simple syrup cool completely.
While the syrup is cooling, squeeze the juice from fresh lemons until you have 1 cup of lemon juice. Strain the juice through a fine-mesh sieve to remove seeds and pulp, if desired.
In a large pitcher, combine the cooled simple syrup, 1 cup of fresh lemon juice, and 3 cups of cold water. Stir well to combine.
Add ice cubes to the pitcher, and if desired, add fresh mint leaves for extra flavor and garnish. Stir gently.
Serve the lemonade in glasses, garnishing each glass with additional mint leaves or lemon slices if desired. Enjoy your refreshing Grand Ole Opry Lemonade!
